Around 50% of people diagnosed with TMAU complain of fecal odor in study - Fecal odor may be sulfur odor according to top expert - concrete evidence of sulfur odor in TMAU

There was a survey done where 90% of people were diagnosed with TMAU, and around 50% complain of a fecal odor. See this video from 29:36: https://www.youtube.com/watch?v=wB3zN5JkD2I&list=PLx3aGntejtWVlKDa22-qsbgdG0bgx0vwK&index=7

The video is put out by credible people (see very start of the video for credentials) and discusses the survey they did. The point is that if you have FBO there is a good chance you actually have TMAU! Only 10% of people with TMAU have a bad fish smell, because TMA smells only smells like fish at very high concentrations. At lower concentrations it smells either simply offensive or garbage-like. Only around 3.5% of people who are diagnosed with TMAU complain about a fish smell, which also means that only around 1/3 of people with a fish odor are aware they smell like fish. For details see: https://www.tesble.com/10.1016/j.amjmed.2011.05.030

Also see my recent posts for more information on this topic. Around 1/3 of people with a malodor complaint turn out to have TMAU. This is based on two major studies, the above linked one and this one: https://www.tesble.com/10.1006/mgme.1999.2885

In my more recent posts, I have been trying to make the case that everyone with a disorder like this should get the TMAU urine test done. This is the definitive one, and not the genetic one, which is more of an optional test. Only 3.5% of people with TMAU complain of a fish odor, and its incidence varies dramatically across populations. It can be reasonablly be estimated that 1/3 people reading this do indeed have TMAU.

If you get a positive TMAU test, then you would be eligable for antibotics to treat and reduce the gut bacteria that create TMA, though this can come at a cost of diarrhea and other consequences. Most importantly, you would know that you are justified in doing the low choline diet. I mentioned a 'positive' TMAU test, but its actually not a binary thing. The severity of TMAU is not deterimined by self reported symptoms. It is determined by the ratio of TMA vs. TMAO in your urine. The more TMA in your urine, the more severe your case is. This can give someone, who is likely noseblind to themselves, a good idea about the actually severity of symptoms. The genetic test does not cover all the possible mutations that can lead to TMAU, and the information about the severity of your case will be much less precise. Top expert says that fecal smells may actually be sulphur (rotten egg) smell:

The late George Preti PhD was probably the worlds leading expert on the subject of human odors and related topics. Here is what he has to say about the idea of fecal smells quite possibly being sulphur smells: (See 55:45 of this video for this information) https://www.youtube.com/watch?v=TZExedNx5Vo

Concrete evidence for sulphur (rotten egg) smells being part of TMAU (at least sometimes)

Link for the medical case report discussed below: https://journals.lww.com/indianjpsychiatry/fulltext/2014/56020/a_rare_case_of_fish_odor_syndrome_presenting_as.18.aspx

I found a medical case study of a young woman who was diagnosed with TMAU via a urine test. Her mother says her husband (father of the TMAU patient) has a terrible rotten egg (sulphur) smell. It sounds like the daughter also has the same odor issue (rotting vegetables). There are some experts who have talked about carriers of TMAU sometimes having symptoms, which is not surprising since they are found not (usually) through a genetic test, but through a urine test showing they have somewhat reduced ability to convert TMA into TMAO. This is clearly a case where both the father and daughter both have TMAU, possibly with both being carriers with symptoms. This is also an interesting example of a likely arranged marriage with someone having TMAU - and the results were not good. There are probably tens of thousands of people in arranged marriages with one person having TMAU, in the developing world right now.
